7/28/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Anthony upon arrival to the home setting. Upon arrival to the home setting, Anthony appeared to be in a good mood. QMHP-C discussed the importance of communication with Anthony and mother. 2. Behavior: Anthony appeared to be content during this session as he was focused and presented cooperative behaviors. 3. Behavior: Anthony was cooperative and did not have any defiant behaviors. 1.Intervention: QMHP-C inquired to Anthony about his current mood and asked Anthony to describe his interaction with his parents today. QMHP-C further asked Anthony to talk about his compliance with following instructions and medication compliance to assist in determining progress towards the current goal and objective. 2. Intervention: QMHP-C thanked Anthony for his honesty and further thanked Anthony for recognizing his lack of progress towards taking his mental health medication on a daily basis. QMHP-C requested Anthony take his medication this evening and requested Anthony’s commitment to take his medication tomorrow. 3. Intervention: QMHP-C asked Anthony about the different roles that he has and if he felt that he was respected in those roles. QMHP-C asked Anthony about the interactions that he has with authority figures and if he felt he respected those people in their roles. QMHP-C encouraged Anthony to begin recognizing how his behaviors affect others and to begin asking himself how he would feel if the roles were reversed and people were disrespecting him. QMHP-C ended session by reviewing interventions with Anthony and encouraging Anthony to provide feedback on the session. 1. Response: Anthony verbalized being fatigued in spite of sleeping the majority of the day and described his mood as okay. Anthony further relayed speaking to his parents after waking up but shared not having any significant conversations with either parent. Anthony added not completing any chores yet however expressed intentions on asking parents about chore completion. 2. Response: Anthony shared not taking his medication yesterday or today however slowly gave his commitment to take his medication this evening after eating dinner. Anthony confirmed during session that he is aware his medication should be taking daily and expressed he is not doing so at this time. 3. Response: Anthony stated that he did not know if he was respecting others in their roles. When asked how he would feel if the roles were reversed and he was being disrespected by others, Anthony stated that he did not know how he would feel.
